Name Change / Replace Wall Document

A credential holder may not use the title in association with a name that is not registered with the Board.

Name change requirements:

  • Your name must be legally changed
  • Legal documentation indicating the name change must be submitted

New wall document requirements:

  • Wall documents are only issued to individuals holding an active CPA license
  • Licensees are only authorized to have one Board issued official wall document
  • You must return your original wall document to the Board before a replacement can be issued
  • If you are no longer in possession of your original, you must provide an explanation before a replacement can be issued

 

Name Change

To change your name with the Board:

  1. Log in to you Secure Access Washington (SAW) account
  2. Access your Board of Accountancy service
  3. Select Name Change/Replace Wall Document
  4. Complete the name change application
  5. Pay the fee, if you selected that you wanted a replacement wall document  
  6. Upload your legal documentation changing your name
  7. Mail your original wall document to the Board at:

Wall Document Replacement

To order a replacement wall document:

  1. Log in to you Secure Access Washington (SAW) account
  2. Access your Board of Accountancy service
  3. Select Name Change/Replace Wall Document
  4. Complete the replacement wall document application
  5. Pay the fee
  6. Mail your original wall document to the Board at:
