Name Change / Preferred First Name Registration / Replacement Wall Document

A CPA may not use the title in association with a name that is not registered with the Board.

Name Change / Preferred First Name Registration

Name change requirements:

  • Your name must be legally changed.
  • Legal documentation indicating the name change must be submitted.

Preferred first name registration:

  • Your legal name must be your name registered with us; however, you can also add a preferred first name that is not your legal name.
  • Legal documentation is not required to register a preferred first name.
  • Your preferred first name will be included on our licensee search along with your legal name making it publicly available for confirmation. It will not appear on CPAVerify.
  • You may use your CPA title in association with your preferred first name once you have registered it with us.

To change your legal name or register a preferred first name with the Board:

  1. Log in to our online services.
  2. Select Name Change.
  3. Complete the name change application.
  4. If you are registering a preferred first name, enter it in the “Preferred First Name” field only (not in the “First Name” field, which is for your legal first name).
  5. Upload your legal documentation if you are changing your legal name.
  6. Pay the fee if you selected that you wanted a replacement wall document. See below for additional information. 

Replacement Wall Document

Replacement wall document requirements:

  • Wall documents are only issued to individuals holding an active CPA license.
  • A wall document is only available in your legal name.
  • Wall document fee of $50 must be paid.

To order a replacement wall document (without a name change):

  1. Log in to our online services.
  2. Select Replacement Wall Document.
  3. Complete the replacement wall document application.
  4. Pay the fee
